Guidelines for After School Programs

The following information is required for all applicants seeking funding for After School Programs:

• Have you applied to The After School Corporation for approval of your program?
• If so, provide a copy of your application and advise us as to what action was taken on your application.
• If not, please provide us with the following information
• The amount of hours per day/days per week/weeks per year, your program is in operation.
• The type of curriculum.
• The total cost per child of your program.
• Does your program operate in an existing school building? If not, where is the location of the program.
• What are the qualifications of those who staff your after school program?
• Number of children attending the program per day.
• Student/Teacher ratio.
• Has your after school program been evaluated by any outside organization? If so, please provide a copy of the evaluation.
• What additional funding have you received for this program. What other applications have you made for funding of this program?
